Manama : In a remarkable display of expertise and teamwork, Shifa Al Jazeera Hospital successfully performed a retrograde intrarenal surgery (RIRS) to remove a pelvi- ureteric junction stone from a 35-year-old Indian patient. Led by consultant urologist Dr. Vishal, the procedure involved the extraction of 9.5-millimeter stone from the left Pelviureteric junction (PUJ).
RIRS, a minimally invasive endoscopic procedure, enables surgeons to access the kidney via the ureter utilizing a flexible ureteroscope. During the surgery, a laser fiber was employed to disintegrate the stones, allowing for the removal of small fragments.
In a span of an hour, the surgery was carried out. “The procedure did not involve any cuts or stitches. As a result, there was no bleeding and that helped in quick recovery. This procedure offers a safe and efficient solution for stone management. We discharged him within 24 hours after the procedure,” Dr. Vishal said.
The surgical team included Consultant Anesthesiologist Dr. Adel Gamal and Anesthesiologist Dr. Azim Palayil.
The patient came to the hospital with severe left flank pain, hematuria, and nausea, symptoms attributed to the pelvi-ureteric junction stone. Timely diagnosis and intervention were imperative to prevent further complications.
The urologist swiftly performed the surgery using a digital flexible ureteroscope to reach the kidney and blast the stone. The procedure was successful, leading to the patient’s stable condition and discharge within 24 hours.
“The use of advanced laser technology in RIRS allows us to effectively remove stones while minimizing risks to the patient. The patient’s swift recovery is a testament to the effectiveness of RIRS. We are pleased with the outcome of this surgery,” explained Dr. Vishal.

RIRS represents a modern approach for treating larger stones, typically between 9mm and 20mm, ensuring patient comfort with pre-procedural anesthesia. Employing imaging technology to pinpoint stone locations, a ureteroscope inspects the ureter/ kidney for the targeted removal or fragmentation of stones using laser technology. Notably, the procedure was conducted with the assistance of Image Intensifiers (C-arm).
The precise and efficient use of advanced laser technology enables the safe removal of stones without causing harm to surrounding tissues, with the subsequent elimination of stone fragments facilitated through urine.
